That's a hospital thing. Most systems have all the data input set for caps lock. I recognize the same error I make often when I've gotta write a quick email where I instinctively go to capitalize the first letter in my name and it's the only thing that comes out in lower case.

Same thing for a lot of people at the insurance company where I used to work. The claims, preauth, and medical audit people often ended up in perma-caps.
